To submit multiple pharmacies at once, download template to enter information into Excel file. Please be sure to complete all fields. Then convert Excel to csv and upload the completed document.

On the Excel document, row 2 and 3 are there for you as an example. When you enter your data, please be sure to REMOVE those rows.

For the website URL column, use main website versus long web address pointing to specific stores (ex. http://www.primetherapeutics.com versus http://www.primetherapeutics.com/mn/123/abc). Use http:// or https:// if those are appropriate for your website. Pharmacies without a website, please use N/A.

Enter yes or no in the American Sign Language column for the following question: Is American Sign Language offered by the pharmacist or a skilled medical interpreter at the pharmacy?

Enter yes or no in the Cultural Competency Training column for the following question: Have the staff in your office completed cultural competency training in the past 12 months?

If yes is entered, provide the month and year of training in the appropriate columns.

For the Chain Code (optional) column, enter the correct chain code with six characters maximum. 

Once the data has been fully entered into the Excel file, save the file as a csv file to upload. Don’t use the CSV UTF-8 (Comma delimited) file type. Before uploading, validate the file using a plain text editor such as Notepad.
